Is there an early access programme for physiotherapy clinics?

No, and it is worth dealing with that first because it is the premise behind most of the pitches clinics are receiving about this channel.

OpenAI publishes two relevant things. An ad policy that sets out which categories may be approved, and an Ads Manager availability list that sets out which countries have self-service access. Neither contains a waiting list for a category that is not approved. There is a notification mechanism, but it is for countries where self-service has not launched yet, not for categories that are restricted.

So there is nothing for a physiotherapy clinic to join, register interest in, or get ahead of. Anyone offering to secure you early access is describing a process that does not appear anywhere in OpenAI's published material.

What happens if a physiotherapy clinic signs up anyway?

You get an account. You do not get ads, and understanding why saves a wasted afternoon.

OpenAI is explicit that account creation and self-service availability are separate things. The Ads Manager country list includes the United States, the United Kingdom, Canada, Australia, New Zealand and the United Arab Emirates among many others, so a clinic in any of those places can complete a signup.

What sits behind the signup is a review. Look at what most of those have in common. They are products, insurance, diagnostics, or institutional care. The two service categories on the list, dental and minimally invasive cosmetic, are both procedural and both bounded: a defined intervention with a defined endpoint.

Physiotherapy is a course of treatment for a physical health condition, which is the exact phrase the restriction uses. That does not make its exclusion permanent or principled, and we would not pretend to know OpenAI's timetable. It does mean a clinic should plan on the current position rather than an imminent change.

Would the placement rule still bite a physiotherapy clinic if the category opened?

Yes, and this is the part that should shape how much a physiotherapy clinic cares about the channel at all.

Conversations about mental and personal health are kept fully ad-free, for every advertiser, whatever their category approval. A prospective patient asking what exercises help sciatica, or whether a shoulder should still hurt after six weeks, is in one of those conversations.

Physical therapist is a Local Services Ads category in the United States. That is a different product from Google Search Ads: it charges per lead, it sits above the search ads, and it carries the Google Verified badge, with Google controlling most of the setup.

The screening is specific and it is the reason the badge means anything. For the health care vertical, which is where physical therapist sits alongside chiropractor, dentist and dermatologist, Google requires a provider check and a practice check, general liability insurance for the practice and professional liability or medical malpractice insurance for the provider, a practice licence and a provider licence at state level, and National Provider Identifier verification.

Two limits to be straight about. That category list is the United States one, so do not assume physical therapist is available in every Local Services Ads country, and Local Services Ads itself runs in a limited set of markets that does not include Australia, New Zealand or the UAE. Spend it where the door is open, which for most clinics means the unpaid side of the same AI surfaces.

Being named in an assistant's answer does not require category approval, does not run into the placement restriction, and does not stop when the budget does.
